Abstract

Background: Otitis media with effusion (OME) is a common cause of childhood hearing impairment, usually resulting from eustachian tube dysfunction and adenoid hypertrophy. Myringotomy and myringotomy with grommet insertion, combined with adenoidectomy, are established surgical treatments for chronic OME, but evidence comparing their hearing outcomes remains mixed.

Objective: To compare postoperative hearing outcomes of adenoidectomy with myringotomy versus adenoidectomy with grommet insertion in children with otitis media with effusion (OME), treated during 2024-25.

Methods:  This retrospective comparative cohort design was used by recruiting 84 children with OME from the medical record of the ENT Department of a tertiary care hospital in Abbottabad from July 2024 to June 2025. OME diagnosed children (record files) who underwent adenoidectomy and myringotomy with and without grommet Insertion were enrolled in 2 groups. Group without gromet and group B with gromet insertion. Postop Pure Tone Audiometry (PTA) data was available to compare one month and three months hearing improvements in both groups. The observation and findings of Data was analyzed on SPSS version 26.

Results: The baseline average pre-operative PTA was almost   similar in two groups. The average PTA results in group A were improved from 35.0 ± 4.2dB preoperatively to 27.0 ± 3.9dB after one month of operation and 31.2 ± 4.1dB after three months post operatively.  Group B showed hearing improvement from 34.0 ± 4.5 dB before surgery to 18.0 ± 3.1 dB at one month and 18.5 ± 3.0 dB at three months after surgery. Group A (27) and group B (37) patients were observed to have hearing improvement of ?10 dB at three months with adenoidectomy and myringotomy and adenoidectomy and grommet insertion, respectively.

Conclusion: Postoperative hearing improvement was better in cases where adenoidectomy with grommet insertion was performed as compared to the cases where adenoidectomy with only myringotomy was done.
